ANORGASMIA

                It is a type of sexual dysfunction in which a person cannot achieve orgasm, even with adequate stimulation. In men, the condition is often related to delayed ejaculation.

Anorgasmia is far more common in females than in males. Anorgasmia can often cause sexual frustration.

Primary Anorgasmia :-

Primary anorgasmia is a condition where one has never experienced an orgasm.

This is significantly more common in women. Women with this condition can sometimes achieve a relatively low level of sexual excitement. Usually there may be no obvious reason why orgasm is unobtainable.

Inability to have orgasm may be related to residual psychosocial perceptions that female sexual desire is somehow 'wrong,' and it stems from the female repression in patriarchal Society.

Secondary Anorgasmia :-

Secondary anorgasmia is the loss of the ability to have orgasms.

Medical Causes :-

It may be caused by

  •    Diabetes (Increased blood glucose level).
  •    Injury to sex organs.
  •    Complications from surgery of sex organs.
  •    Injury to hip region.
  •    Imbalance of hormones (various chemical which control various body functions)
  •    Removal of uterus (womb)
  •    Back bone injury,
  •    Childbirth trauma
  •    Cardiovascular disease,
  •    Anti-depressants (SSRI)
  •    Opiate addiction (Heroin)
  •    Alcohol

Prostatectomy :-

Secondary anorgasmia is close to 50% among males after removal of prostate. This is generally caused by damage to the primary nerves serving the penile area, which pass near the prostate gland. Removal of the prostate frequently damages or even completely removes these nerves, making sexual response difficult.

Situational Anorgasmia :-

  •    People who are orgasmic in some situations may not be in others.
  •    A person may have an orgasm from one type of stimulation but not from another
  •    May achieve orgasm with one partner but not another
  •    Have an orgasm only under certain conditions or only with a certain type or amount of foreplay.
  •    Fatigue,
  •    Emotional concerns,
  •    Feeling pressured to have sex when person is not interested
  •    Partner's sexual dysfunction.

TREATMENT IN WOMEN :-

   To remove factors causing Situational Anorgasmia.

   In case of female situational anorgasmia

   1: Couple has to incorporate clitoral stimulation before intercourse,

   2: Couple has to use female-above position as it may allow for greater stimulation of the clitoris and it allows the woman better control of movement.

In the case of women suffering from psychological sexual trauma psychosexual counseling is sufficient.

In case of women suffering from anorgasmia without any known cause blood results for hormonal levels, Thyroid function and Diabetes are helpful. A neurological work-up is needed to determine the degree of nerve damage.

TREATMENT IN MEN :-

The chemical cabergoline is effective in 1/3rd of anorgasmic subjects.
